What to Do if You Lost Your Residence Card or Passport

If you lose your Residence Card:

If you lose your Residence Card, you are required to apply for reissuance. First of all, please submit a "lost report" to the nearest police office or police box, get a certificate of loss report, a certificate of theft report, and a certificate of disaster, and apply for reissuance at the Regional Immigration Services Bureau with jurisdiction over your place of residence.

Applicant: Person who has lost their Residence Card

Required documents:
  1. Application for reissuance of a Residence Card (Download this application form on the Immigration Services Agency of Japan website.)
  2. Your photo
  3. A certificate of loss report, etc. (obtained at a police office or police box)
  4. Your passport (just to present)
Processing fee: No charge

Note: Once your Residence Card is reissued, please go to the office in charge at your campus to present it.

If you lose your passport:

  1. Submit a "lost report" to the nearest police office or police box, get a certificate of loss report, a certificate of theft report, and a certificate of disaster.
  2. Apply for reissuance with the certificates listed in the above 1. at the consular section of the embassy of your nationality in Japan.
  3. Conduct the procedure for "Transfer of Endorsement” with the reissued passport and the certificates listed in the above 1 at the Regional Immigration Services Bureau.
