Issuance of certificates after graduate-program completion

Various certificates concerning graduate school alumni can be applied for by visiting the section in charge on each campus or by sending an application by post. Applications by phone, FAX or email cannot be accepted.

To protect personal information, the applicant is requested to show (or, when applications are made by post, enclose a photocopy of) his/her ID to prove that he/she is the alumnus/alumna for whom a certificate is being applied for, at the time of certificate issuance.

If someone other than the alumnus/alumna (applicant) for whom a certificate is being applied for (hereinafter, “the proxy”) applies in place of the applicant, the proxy is requested to show a letter of proxy from the applicant, the applicant’s ID (or its photocopy), and the proxy’s own ID. (When application is made by post, the letter of proxy, a photocopy of the applicant’s ID, and a photocopy of the proxy’s ID must be enclosed with the application documents.) No certificates can be issued unless the above-designated documents are shown or submitted.

  1. The processing time for issuing certificates is as follows:
    Certificate of Completion (in Japanese) and Academic Transcript (in Japanese) in the University’s designated format: Same day (Visit the counter at least 30 minutes before closing time.)
    Certificates other than the above: About one week after application
    Apply as early as possible so that you can receive the certificate in good time. (The above-specified periods do not include delivery times in the case of application by post.)
    Please note that the number of days taken to issue certificates may differ from the above during long breaks, campus festivals, etc.
  2. If certificates must be issued in designated forms, be sure to bring (or enclose for application by post) the designated forms at the time of application.
  3. If you are applying for the Certificate of Academic Standing (for Teaching Certificate), clearly indicate which of the latest, previous and older revisions of the Educational Personnel Certification Act the certificate must comply with, and in which course your academic standing must be certified (“junior high school social studies,” “senior high school civics,” etc.). In addition, enclose photocopies of both sides of your Basic Teaching Certificate with the application documents.
  4. If you want the issued certificate to be sent in a hermetically sealed envelope, ask the section in charge. (If you apply by post, clearly indicate your request in writing.)
  5. The personal information that applicants provide for identification will be registered in the alumni database with each applicant’s consent. Alumni data is managed and safeguarded appropriately and used only within the scope of enabling the Toyo University Incorporated Educational Institution to contact alumni and will never be used for other purposes. After certificates are issued, ID documents submitted by applicants will be destroyed under the University’s responsibility.

Acceptable ID

Acceptable ID are listed below. Applicants (and proxies) are requested to show (or send a photocopy of) one of them.

  • Driver’s license
  • Passport
  • Health insurance card
  • Basic Resident Registration Card
  • Individual Number Card (only its front side)
  • Residence Card (both sides)

* Applicants (and proxies) must show (or send a photocopy of) their ID document every time they apply for certificates.

* If you are to send a photocopy of an Individual Number Card as an ID paper, photocopy only the front side of the card because the card holder’s Individual Number is shown on the back side of the card.

* If the applicant’s family name has changed since completing the graduate program, in addition to the basic ID document (any of the above-designated documents), any of the following documents must be presented: an official copy of the applicant’s family register (full or extract); the applicant’s Individual Number Card; an official copy of the applicant’s certificate of residence; or the applicant’s driver’s license showing the former family name. (A photocopy of an official copy of the family register or certificate of residence will not be accepted. The proxy does not have to present any of these documents.) Please note, however, that certificates will be issued under the applicant’s name at the time of completion. (Certificates cannot show the applicant’s name at the time of application.)

* If the applicant’s given name has changed since completing the graduate program, in addition to the basic ID document (any of the above-designated documents), the applicant must present an official copy of his/her family register (full or extract). (A photocopy of an official copy of the family register will not be accepted. The proxy does not have to present an official copy of his/her own family register.) Please note, however, that certificates will be issued under the applicant’s name at the time of completion. (Certificates cannot show the applicant’s name at the time of application.)

1. How to apply at the counter

   Issue the designated application form using a certificate-issuing machine on campus, fill it in, and submit it together with the following documents at the counter of the section in charge.

If you cannot print it out, write the following information on any sheet of paper.

If you need to have your enrollment period certified in order to receive a pension or for other purposes, have a Certificate of Completion issued for a fee.
(The Certificate of Completion also indicates the enrollment period.)
* Please note that the Certificate of Enrollment Period issued by Toyo University is intended to certify the enrollment period of a former student who withdrew (or was expelled) from the University.

Necessary information

① Names of the graduate school and program (“Master’s Program” etc.) at the time of enrollment
② Student ID number
③ Name (with its katakana transliteration)
④ Date of birth
⑤ Year and month of completion, withdrawal with fulfilled program requirements, etc.
⑥ Contact address and phone number (mobile phone number at which the applicant can be easily reached during the daytime)
⑦ Name of the necessary certificate and the necessary number of its copies
⑧ Use and where the certificate will be submitted (only if you are applying for the Certificate of Academic Standing or any certificates in English)
⑨ Name in alphabetical letters (just as shown in the passport; only if you are applying for any certificates in English)
⑩ If the applicant wishes to have his/her personal information not registered in the alumni data,* declaration of his/her wish

Return envelope

Envelope larger than 23.5 × 12 cm, with the address and name of the recipient written and a postage stamp affixed
* Please be careful about the revision of postage and difference in postage due to the weight of postal matter.
• If you want the return envelope to be sealed with the university’s official stamp over the envelope flap, enclose an envelope larger than 33.2 × 24 cm.

Fee

Only postal money orders are accepted. (Do not write anything on the postal money order.) Cash or postage stamps are not accepted.
* Be sure to check that the amount is exact before you enclose the postal money order.

Photocopy of an ID document of the applicant (alumnus/alumna)
* In the case of the Individual Number Card, a photocopy of only the front side.
* In the case of the Residence Card, a photocopy of both sides.
* If the applicant’s family name has changed since completion, in addition to a photocopy of the above-designated basic ID document, any of the following documents showing the applicant’s former family name must be provided: an official copy of the applicant’s family register (full or extract), a photocopy of the front side of the applicant’s Individual Number Card, an official copy of the applicant’s certificate of residence, or photocopies of both sides of the applicant’s driver’s license. (A photocopy of an official copy of the family register or certificate of residence will not be accepted. The proxy does not have to present any of these documents.)
* If the applicant’s given name has changed since completion, in addition to a photocopy of the above-designated basic ID document, an official copy of his/her family register (full or extract) must be provided. (A photocopy of an official copy of the family register will not be accepted. The proxy does not have to present an official copy of his/her own family register.)

Certifies that a Doctoral Program student who withdrew (with the expiration of his/her enrollment period) after fulfilling overall credit requirements excluding the thesis review and final exam during his/her enrollment.
Please note that this certificate is different in the contents and the fee from the Certificate of Earned Credits used to obtain qualifications. Be sure to write the proper name of the certificate you need in the application form. (Write in the “Other” field.)

Certificate of Academic Degree

200 yen

Certifies that a graduate student earned credits, received research guidance, passed the thesis review and final exam, and obtained an academic degree at his/her graduate school.

Certificate of Enrollment Period

300 yen

Certifies the enrollment period at a graduate school of a student who withdrew or was expelled midway.

Certificate of Academic Standing
(former Certificate of Earned Credits Necessary for Teaching Certificate)

300 yen

Certifies that a student earned the necessary credits for obtaining a Teaching Certificate.

Character Certificate (in the University’s designated format)

200 yen

Certificate in the University’s designated format, written by the student’s academic supervisor based on the supervisor’s general observations about the student. If your academic supervisor is already retired, consult us in advance.

Certificate of Completion, and Certificate of Completion of Designated Courses for Certified Social Workers

300 yen

Certificate intended for alumni who take exams for certified social workers.

* If you are applying for the English Certificate of Completion, English Academic Transcript or English Certificate of Withdrawal with Fulfilled Credit Requirements, clearly indicate your name in alphabetical letters (just as shown in your passport), where you will submit the certificate, and its use.

* The fee amounts shown here are unit amounts for one copy of a certificate as of April 1, 2019. For information about certificates other than the above-listed ones, consult the section in charge.
* For information about fees for certificates in format other than the University’s designated format, consult the section in charge.
* The fee for any type of certificate (in Japanese) concerning non-matriculated students is 300 yen.
* The fee for the Certificate of Academic Standing in compliance with the latest and previous revisions of the Educational Personnel Certification Act is 300 yen per course, while the fee for the Certificate of Academic Standing in compliance with the older revision of the Act is 300 yen regardless of the number of courses. (However, the issuance of the Certificate of Academic Standing used to obtain the Teaching Certificate for special needs education requires an additional fee of 300 yen.)

  5. The personal information that applicants offer to us for identification will be registered in the alumni data with the consent of the applicants. The alumni data is managed and safeguarded appropriately and used within the scope necessary for the Toyo University Incorporated Educational Institution to contact alumni. The alumni data will be never used for other purposes. After certificates are issued, ID copies submitted by the applicants will be destroyed under the University’s responsibility.
