Issuance of Certificates after Graduation

Requests for certificates may be submitted by mail or in person at the Education Affairs Section of Each Faculty on Hakusan Campus.  Please note that requests cannot be made through telephone, fax or e-mail.

For the purpose of protecting personal information, Toyo University requires applicants for certificates to present certain documentation to verify their identity when issuing certificates (or to enclose a photocopy of their identification when applying via postal mail). If a person other than the applicant (“proxy”) files an application, it is necessary to present a letter of proxy, identification of the applicant (photocopy acceptable), and identification of the proxy (in the case of application via postal mail, photocopies of identification of the applicant and proxy need to be enclosed). If any of the required documents are missing, your requested certificate will not be issued.

 

About Identification Documents

For identification, prepare one of the following documents.

Driver’s License , Passport , Health Insurance Card , “My Number” Card , Basic Resident Register Card , Residence Card

* Identification must be presented each time an application for certificates is submitted (a photocopy of identification must be enclosed when applying by mail).

* When mailing a photocopy of a “My number” card as identification, since the individual number is printed on the back of the card, make a photocopy of only the front of the card and send it to the appropriate office.

* For an applicant who changed his/her last or first name after graduation, in addition to one of the above-listed identification documents, an original copy (photocopy not accepted) of his/her family register (or extract) is also required (not required for identification of the proxy). Please note that certificates will be issued with the applicant’s name at the time of graduation (certificates cannot be issued with the applicant’s current name).

 

1. How to apply at the office

Print an application form from a certificate-issuing machine on Hakusan Campus. Submit the completed form together with the following required documents to the Education Affairs Section of the Faculty from which the applicant graduated.

Print a letter of proxy form the Toyo University website and fill in the necessary information.

* If the letter of proxy form cannot be printed out, a letter of proxy may be prepared in any format, as long as it contains the applicant’s signature and seal and a description to the effect that the applicant entrusts application to the proxy.

* If the applicant does not want his/her personal information to be registered in the alumni database, indicate to that effect in the letter of proxy.

 

2. How to apply by postal mail

Send the following documents by post to the Education Affairs Section of the Faculty from the applicant graduated.

Please download and print an application form from the Toyo University website, and fill in the necessary information.

* If you cannot print the application form, write all the required information listed below on an appropriate sheet of paper.

c. Student ID number

d. Name (with furigana attached)

e. Date of birth

f. Year and month of graduation

g. Current address and phone number (reachable during office hours)

h. Types of certificates and number of copies required

i. Purpose of use (only when requesting Certificate of Academic Standing or English-version certificate)

j. Organization to which the certificate will be submitted (only when requesting Certificate of Academic Standing or English-version certificate)

k. Name in English (as printed on passport; only when requesting English-version certificates)

* If you do not want your personal information to be registered in the alumni database, indicate to that effect.

Return envelope

Please enclose a self-addressed stamped envelope.

* Enclose an envelope (23.5 cm high x 12 cm wide) or larger.

* If you wish your certificate to be placed in a sealed envelope, enclose an envelope (33.2 cm high x 24 cm wide) or larger.

Issuing fees

Only fixed-amount postal money order of Japan Post Bank (Teigaku kogawase) is acceptable. Cash or stamps are not accepted.

* Be sure to enclose a postal money order in the exact amount, neither too much nor too little.

* No need to fill out the postal money order.

A Photocopy of identification of the applicant

* In the case of “My number” card, make a copy of only the front of the card.

* In the case of residence card, make copies of both front and back sides of the card.

* Fees indicated above are on a per copy basis, as of April 1, 2017.

* For certificates other than those listed above, contact the Education Affairs Section of the relevant faculty.

* For Certificate of Graduation, Academic Transcript and Character Reference in a format other than the University-designated format, you will be charged 300 yen per copy.

* In the case of non-matriculated students, a fee of 300 yen will be charged uniformly for any certificates in Japanese.

* For Certificate of Academic Standing, when the “Shimpou”(新法) is applied, the fee is 300 yen per subject; when the “Kyuhou”(旧法), or the “Kyukyuhou”(旧々法) is applied, the fee is uniformly 300 yen regardless of the number of subjects (however, for a certificate for special needs education, the fee of 300 yen is separately required).

 

4. Number of days required for issuance

The number of days required for issuance depends on the type of certificate. So please make an application well in advance.

● Certificate of Graduation (Japanese) / Academic Transcript (Japanese): On the same day

● Certificates other than above: About one week

* In the case of application by postal mail, please allow additional time for mail delivery.

* Note that it may take longer to issue certificates during long school holiday periods and the University festival period.

 

5. Others

(1) If there is a specified form for certification, be sure to bring it to the office (enclose it in the case of application by postal mail).

(2) A Character Reference can be issued only to those who submitted a Character Report (Jinbutsu Chosasho) while enrolled in the University.

Please note that the Character Report is valid for three years after graduation.

(3) In the case of academic transcript, indicate the applied law (“Shimpou”, “Kyuhou”, or “Kyukyuhou”) and the subject (Lower Secondary Social Studies, Upper Secondary Civics, etc.).

(4) If you would like your certificates to be placed in sealed envelopes, please indicate so in the application form. Certificates will be supplied in sealed envelopes prepared by the University.

If you have a specific request for how certificates should be sealed in envelopes (e.g. one certificate per sealed envelope), please clearly indicate your request.

When you apply via postal mail, be sure to indicate your request about envelope sealing in the relevant section on the application form. When you apply at the office, please also communicate your request directly to the person receiving your application.

(5) Your personal information provided for identification purposes will be registered in the alumni database, upon your approval.

The personal data of alumni will be handled and managed appropriately and will be used only for the purpose of Toyo University contacting you. All submitted identification documents will be shredded and discarded after issuance of certificates.
